Where Can I Find My Number?
You can find the required number on your statement. Please refer to the examples below for guidance.
Patient Statement
On your Patient Statement, look for the MRN located near the top of the statement.

On your Patient Statement, look for the ACCT. # located in the payment information section.
How It Works
Select Payment Type
Choose Patient Payment or Medical Records Payment.
Enter Your Number
For Patient Payment, enter the required number shown on your statement.
Complete Your Payment
Click Make a Payment to continue to the secure payment portal.
Secure Online Payment
You will be redirected to our secure payment portal to complete your transaction. Your payment information is securely processed through our trusted payment provider.
